A way to request extra information from the customers. Forms do not affect the price for the package being booked.

There are 4 field type options; Text Box, Radio Button, Drop-down List and Check Box. Each can be set as required or optional.

Adding a form to a service
From the dashboard open the Forms and click the ‘New’ button., fill out the details and save & close.

The database identification number.

Publish or un-publish the form.

Yes or No.
Yes: the customer will not be able to proceed with the booking until entering an answer.
No: the form field is optional.

Ask the question that will get the information you need.
Example – How did you hear about our services?

Field Type
Select the type of form field you wish to use.

If the chosen field type requires answers fill them out with a ‘|’ between each option.
Radio Box Example – Television | Newspaper | Internet Search

Tick the services to which the form you be applied to.

To adjust the order of the listed form elements click the ‘Ordering’ title of the main table. Arrows will appear and then you can adjust as required. The table order will affect how the public see the order of the forms.
